Conditions that apply: There is an imminent risk of out-of-home placement as a result of treatment issues. Less intensive services have been tried (CCT or other) and either: Services have not decreased (number of stays or length of stay in RTC) ; or services were not successful in managing symptoms or maintaining daily functioning of the child/adolescent. Either the child/adolescent is being discharged from a more intensive level of care (i.e. hospital or residential care) and CCFT is appropriate to support the return to the community; or behaviors have escalated in the past 30 days showing significant changes in school, home, or community functioning.
