Description: Provides in-home services to famlies (1.) with a member who has undergone inpatient mental health treatment and (2.) are at risk of becoming unstable. Services are also available to children at risk for psychiatric hospitalization. The program seeks to decrease the frequency of repeat hospitalizations for mental health issues. Case management services are available in-home and by phone 24 hours per day, 7 days per week. The following are issues addressed by the program: conflict resolution, problem solving, impulse management, parenting, budgeting, social skills development, daily living skills, and learning to use community resources.

Conditions that apply: For children ages 3-18 in Hamilton or surrounding counties. TennCare enrolees need to have recieved a TPG rating of 2 from a local mental health center. (The agency can help arrange a visit to a local mental health center.) Potential clients must not be currently suicidal or homicidal, and must have a diagnosed mental health condition.
